Surreal Awards was once a highly-rated awards program.  It's closed now, but the excellent tutorials on designing a web award are still there -- at least for now.  (At some point, this will probably disappear.  Take advantage of the tutorial while you still can!)

Korea Award offers free award templates by permission of the webmaster.

Vision Site Design Awards offers free graphics templates for your exclusive adoption (once you select a set, they'll won't give them to anyone else).

World Wide Web Awards has some award templates available for free to legitimate awards programs.
